Forest Avenue Site

The Forest Avenue Residence has been established as an extension to the community based CCNP continuum of service, which includes case management, in-home services and respite supports. The residential program provides a therapeutic environment for youth with a dual diagnosis between 12-18 years of age. The program consists of six (6) beds with the capacity to provide short-term stabilization residential service for up to two (2) youth, with an additional four (2) beds for planned respite support. The two stabilization beds may be accessed to support transitional planning for youth returning from more intrusive placements.

The program objective is to assist with the development of and support  the capacity for youth to reside within their family homes.
