Children with Complex Needs Partnership (CCNP)

This program addresses the needs of children with complex needs including those with a dual diagnosis, and is provided in conjunction with its service partners, with Lynwood Hall providing In-home Services support and Case Management and family support, with Rygiel Supports for Commuity Living, providing coordination of accommodation options, including respite.

The focus of the In-home Service is to provide intensive in-home support to families and their children/youth. The support can be provided in-home,school, and/or within the community. The Case ManagementService provides support for parents in coordinating services, navigating the service system, and in assisting with transitioning between services.  Both services are available to families with children and youth between the ages of 0-18 with a complex mental health challenge or those with a dual diagnosis. The service is available to children and youth who may be returning from an out of home placement, or who are at risk of an out of home placement.

The program objective is to deliver an intensive service to children and youth with either a dual diagnosis, or complex mental health need. The service model includes a process of planned, multidimensional, community based services and informal supports.
